He was attached to the 3rd Battalion Sherwood Foresters at East Boldon. He died in 1st Northern General Hospital at 5.30pm on 10th September.

"Deceased died from injuries received by accidentally slipping and falling from the draw bar of a waggon [sic] and was run over by the wheel of the said waggon at East Boldon on Friday 10th Sept".
