Sgt Meachen was killed when Avro Lancaster III ED470 QR-W of 61 Squadron was lost on a mission to bomb the Luftwaffe night fighter airfield at Munster. The Lancaster crashed into the River Waal, near Arnhem, with the loss of six of the seven crew members; only the rear gunner survived.
