Plaque 4½ inches high x 5½ inches wide, very plain, set on a plain wood pattress. Lettering is in sans serif capitals throughout.

1. This church combined with St. Mary (demolished in 1967).
2. When the church closed, this was moved first to Blackhall St Andrew's then to Hesleden Community Centre. When the Community Centre closed, the plaque was taken into the Primary School.
