Flight Lieutenant Simpson was Air Bomber aboard Liberator VI KH254 of 99 Sqn, which collided head on with another 99 Sqn Liberator KG974 whilst in a formation-forming circuit near Noakhali at 07.06 on January 21st 1945. The crews from both aircraft were all killed.
Also killed in the same incident but in the other aircraft was Ft/Sgt. P.B. Haile
