Sgt. Small was serving with 28 O.T.U. He was a crew member of Wellington IC R1183 which took off from Castle Donington on a night training exercise. It crashed after take-off because the wing flaps dropped fully down causing the aircraft to stall. One man was killed, four others, including Sgt Small, were injured, but he died later.
